Rather, reason means that persons are capable of determining for themselves what is right and wrong, capable of "instructing themselves" to do the right thing, and capable of understanding why, depending on what they choose to do, they are morally praiseworthy or blameworthy. " To be capable of autonomy is to be capable of independently (1) developing principles of behavior, (2) recognizing that these principles should guide one's own behavior, and (3) evaluating one's own and others' behavior with reference to those principles.
HCPs may be tempted to reason thusly: I will find out what the patient's values, goals, and interests are; then I will respect them by choosing the medical regimen that will most likely enhance or protect these values. This reasoning is problematic for a number of reasons. Most basically, the approach is inappropriate because HCPs are almost always operating with an information deficit about the patient's full set of values, interests, and goals.
